If you are a young person aged 12- 25, we’d like your permission to ask you a few brief screening questions to get a sense of whether this study might
be a good fit for you. These questions will help us assess whether it would be appropriate to invite you for an appointment to discuss the study
further. Our goal is to ensure your time isn’t wasted by attending an appointment if the study isn’t suitable for you. The screening questions focus
on your mental health and will take approximately two minutes to complete.
If you are a parent, carer, or clinician looking to refer a young person or learn more about the study, we will ask for your contact details so we can
get in touch with further information.

Your responses will only be used for the purpose of this research project and will only be accessed by research staff involved in this project. All information collected will be stored electronically on secure servers that are password protected. The collected information will be kept strictly confidential for as long as it takes us to assess if you are eligible, up to one month. Minimal identifying information will be kept until end of study recruitment. This includes your initials, month and year of birth.
